Criteria for preserving posterior cruciate ligament depending on intra-operative gap measurement in total knee replacement

چکیده
OBJECTIVES Because posterior cruciate ligament (PCL) resection makes flexion gaps wider in total knee replacement (TKR), preserving or sacrificing a PCL affects the gap equivalence; however, there are no criteria for the PCL resection that consider gap situations of each knee. This study aims to investigate gap characteristics of knees and to consider the criteria for PCL resection. METHODS The extension and flexion gaps were measured, first with the PCL preserved and subsequently with the PCL removed (in cases in which posterior substitute components were selected). The PCL preservation or sacrifice was solely determined by the gap measurement results, without considering other functions of the PCL such as 'roll back.' RESULTS Wide variations were observed in the extension and flexion gaps. The flexion gaps were significantly larger than the extension gaps. Cases with 18 mm or more flexion gap and with larger flexion than extension gap were implanted with cruciate retaining component. A posterior substitute component was implanted with the other cases. CONCLUSIONS In order to make adequate gaps, it is important to decide whether to preserve the PCL based on the intra-operative gap measurements made with the PCL intact. Are lesions of the posterior cruciate ligament predictable before knee arthroplasty? A histological study of 434 ligaments in osteoarthritic knees.
Despite the number of publications which have dealt with posterior cruciate ligament (PCL) retaining total knee arthroplasty, few studies have addressed the histology of the PCL. Based on these, the use of some predictive factors for lesions of the PCL has been suggested, as a decisive argument to substitute the ligament or not. The objective of this study was to assess the value of some predic...

We have developed a new tensor for total knee replacements which is designed to assist with soft-tissue balancing throughout the full range of movement with a reduced patellofemoral joint. Using this tensor in 40 patients with osteoarthritis we compared the intra-operative joint gap in cruciate-retaining and posterior-stabilised total knee replacements at 0 degrees , 10 degrees , 45 degrees , 9...

We have examined the relationship between the size of the flexion gap and the anterior translation of the tibia in flexion during implantation of a posterior cruciate ligament (PCL)-retaining BalanSys total knee replacement (TKR). In 91 knees, the flexion gap and anterior tibial translation were measured intra-operatively using a custom-made, flexible tensor-spacer device. The results showed th...
متن کاملThe effect of distal femoral bone cut on flexion gap in cruciate retaining total knee arthroplasty -A cadaveric study-
INTRODUCTION: Proper bone cut and soft tissue balancing are the most critical procedures in total knee arthroplasty (TKA). A gap balancing technique is commonly used in TKA to create an equal extension and flexion gap. However, obtaining a flexion gap sufficiently large for the extension gap in cruciate retaining (CR) TKA is sometimes difficult and the flexion gap in CR TKA tends to be smaller ...

Achieving deep flexion after total knee replacement remains a challenge. In this study we compared the soft-tissue tension and tibiofemoral force in a mobile-bearing posterior cruciate ligament-sacrificing total knee replacement, using equal flexion and extension gaps, and with the gaps increased by 2 mm each. The tests were conducted during passive movement in five cadaver knees, and measureme...
